- AAHB member cost for registering for CHES credits: $30
- Non-member cost for registering for CHES credits: $50

Either print and complete after attending each session or stay green and complete the forms electronically.
* E-mail the completed evaluation form to me at: jsommer@bgsu.edu or mail to the address below by Friday, March 27, 2016. Clearly indicate if you are requesting CHES or MCHES and provide your number.
